Is medscape a reliable source for explanations? i was trying to find the pathophys behind why platelets are dysfunctional in uremic patients.

If your institution has a subscription, you might want to try UpToDate (http://www.uptodate.com/contents/platelet-dysfunction-in-uremia) or if you want actual studies then NCBI/Pubmed for journal articles.

UpToDate has largely superceded everything else, but otherwise, yes, MedScape is fine as they use more or less the same sources.

Platelets are dysfunctional in uremia because they don't degranulate and hence aggregation and adhesion are affected. Here's a discussion about platelet dysfunction in renal failure in general.

I use Medscape all the time for reference.
They are very reliable as compared to Wikipedia which is good for a quick look.
