Mental status examination and psychiatric Hx

Hi guys,

I was wondering whether we are expected to do a mental status examination (not MMSE): general appearances, psychomotor behavior, mood and affect, speech, cognition, thought patterns, level of consciousness.

Also any suggestions on a resource to best structure my psychiatric Hx. I'm already using FA, but its not great.

G- Grooming/ Appearance
J- Judgment
M- Mood
C- (COMA: Concentration, Orientation, Memory, Attention)
A- Affect
P- Perception (PhD: Paranoia, Hallucination, Delusion)
S- Suicidal/Homicidal ideation

It's missing thoughts (content and form) So I added T next to M: SIGJ MTcf CAPS. Thank you for this mnemonic helps me a lot.
