Let's say the hospital that I am volunteering in is considered to be undeserved according to MSAR. Should I mention this anywhere in my application, or will medical schools already know? Thanks.
The MSAR classifies hospitals in terms of being underserved? I'm not sure that stating that a hospital is in an underserved area would add anything to your narrative.

Unless you were using this experience as support for your commitment to working with low income/underserved populations, no. The hospital itself being in an underserved area really means nothing unless you specifically want to serve this population as a physician and use it as a way to "show" why you want to be a doctor.
You should only speak about underserved if the fact that it was underserved played a role in your decision to volunteer there. It just happening to be underserved doesn't count. It'd be tacky to mention it.
