Midway threw my 3rd year of college: need advice for PT school

First off, what is your backup plan if PT school doesn't work out? How does a bachelors in Worksite Health Promotion fit into that? That's something you need to ask yourself.

As far as taking classes at a community college, I wouldn't suggest doing that for too many prereqs. If you go to PTCAS.org and look at their school list requirements, some schools require that you took classes like anatomy at a 4-year school (the majority of them, as far as I can tell, don't mind it, but you're limiting your options).

Also, although the grade-replace program may work for your school and your bachelors degree, the original grade and it's impact on GPA remain there on grad school application.
The good news is that a lot of schools place a lot of emphasis on the last 60 credit hours you took. There is still plenty of time to build your application to PT school. Doing so includes more than just GPA. Make sure you're getting varied volunteer experiences, make plenty of time to study for the GRE, and also remember you need references and EC's.
