Since the number of applicants to each field is varied each year, I was wondering if it was possible to scramble into a different field if you didn't match at first. For example, if I applied to ortho but didn't get it, but a gen surg urology prelim year program wasn't filled, could I try scrambling into it? What does the army do when they don't fill certain positions?

And what if all the ophtho slots happen to be open...
Seriously, the GMESB fills all the slots during their initial meeting. People are given their second, third choices, etc. as the available slots allow. Applying to multiple specialties is usually not helpful to getting a selective residency.
