Both of these questions are so region-specific as to be pretty much useless nationally.

Where I'm at, in-house moonlighting is about $60/hour. Outside is $80-$110/hour. The best place to find opportunities is through senior residents. Your residency program should also have a method for announcing moonlighting opportunities, as any jobs open to residents will typically contact local residencies.
