More licensure questions

Hello,

Long time reader, first time poster (this site was extremely helpful to me over the past year, but I always found the answers I needed in the archives and never felt that I had much to contribute at the time). I've been looking over the threads about disclosing information on practice license applications and had two questions.

First, I failed step 1 twice. I am not aware of ever being placed on academic probation. I also have 1 pass-with-remediation on my transcript in a basic a sciences class. Are these things that should be disclosed in the application, or just included in the school and USMLE transcripts?

Second, I was diagnosed with ADHD during this time period and for a while treated it with methylphenidate, but am not currently taking any medications (instead I took up daily exercise and learned better study habits and to force myself to pay attention on rounds, etc.). Is this something I should disclose in the section about mental disorders?

I should mention that my residency program is in Texas, which I understand might have more stringent licensing criteria.

Every application for licensure has specific questions they ask you. Most involve whether you have been hospitalized for certain disorders, drug addictions, etc. they also ask if you have ever been suspended by a hospital board, had your DEA revoked, etc. If you have ever given up your hospital priviliges in order to avoid formal penalty, etc.You just need to answer the questions the best you can and provide any explanations. Many applications will have you check how many times you took each exam. There is no point in trying to skirt any issue, just fill out the paperwork the best of your knowledge and turn it in. Agreed TX license is difficult to obtain and takes about 10 months to get. I suggest you use FCVS for all your verification. It will be less difficult if you decide to get more than one license.
