More rec letter questions

I'm a little confused re: letters:

Besides the Dean's Letter and the 3 standard letters of rec (1 from core, 2 from ophthalmologists), can we send in an extra "bonus" letter? Also, although CAS recommends a letter from a core rotation, is it advisable to send in three ophtho letters?

As far as I understand, the "bonus" letter is meant to allow students doing later ophtho rotations to get a letter from those. I think the only stipulation is that it has to be from an ophthalmologist, and I doubt they check when you did the rotation with the letter writer.

As far as three ophtho letters, my advisor, the former program director of my school's program, actually advised me to only submit one ophtho letter. I'm using two, but I think three might be too much. They want to see that you're successful in other fields (especially med and surg) too.
