Most common cause of sudden cardiac Death: Can someone explain how AV stenosis causes sudden cardiac death in ?

(1) AV stenosis (most common cause)

(2) Cardiomyopathies (hypertrophic cardiomyopathy is the most common cause)

(3) Wolff-Parkinson-White syndrome

2005 paper said the following: 1. An abnormal Betzold-Jarisch reflex (stimulation of left ventricular baroreceptors may lead to arterial hypotension, a fall in venous return and consequent bradycardia)
2. Ventricular tachyarrhythmias (inappropriate hypotension and a low cardiac output provoke coronary hypoperfusion, in patients who already have a predisposition through left ventricular hypertrophy)
3. Atrioventricular conduction disturbances (there is a contiguity between valvular-perivalvular calcification and the His-Purkinje system [there is also a high prevalence of conduction abnormalities in patients with AS])

Not sure if there is anything more recent but these sound fairly plausible.

If your talking about aortic valve stenosis (and not the av node)
(1) AV stenosis (most common cause)


(2) Cardiomyopathies (hypertrophic cardiomyopathy is the most common cause)

(3) Wolff-Parkinson-White syndrome
1) I would think it's just a problem of blood not being able to shot out into the aorta and perfuse the brain, heart, and kidney. The brain fail after 20 minutes of ischemia, the heart and kidney would fail after 6 hours. You could classify 20 minutes as pretty sudden.

2) HOCM - septum is pushing on the mitral valve usually, which would lead to not enough blood being pushed out. This would be exacerbated by aortic stenosis and lead to #1

3) WPW - you have a problem with depolarization, the AV node is bypassed (goes from SA node to Purkinje fibers), which would lead to problem with contraction and lead to #1
