The MSAR is an AAMC publication and thus it does not have the AACOM schools. AACOM does have an informative publication which you can view online for free or you can order it here
 
I just downloaded the AACOMAS version of the MSAR from the their website and printed it off. For free. That was nice.

I was a little disgruntled that the MSAR doesn't include DO schools though. They're all "medical schools" so why can't they just make one publication? And they do include the caribbean schools (sp? . . . I'm having a hard time with spelling today!), and canadian schools, so why not DO school. I know. I know. The big ominous split between AAMC and AOA . . . what a pain, huh.

MSAR is for the AMCAS application service. This does not include all allopathic medical schools and has nothing to do with AAMC vs. AOA.

The Texas medical schools both allopathic and osteopathic are also completely separate. They use the TMDSAS application service.

When I applied for medical schools, I had to fill out 3 separate applications, including AMCAS, AACOMAS, and TMDSAS.
