You don’t need scholarships or papers to be competitive for MD/PhD. You need a demonstrated interest in pursuing a scientific career, letters that will back up your ability to do science, and impressive numbers. You have all of those things. You will be fine with a good school list.

Is it normal for MD/PhD applicants to have worked in only one lab, thus I'll only have one letter from a faculty with whom I did research?

Also, which schools would you recommend given my qualifications?

Idk about "normal", but its fine, it doesnt really matter. The longer the commitment the better, in my opinion.

I think you would be competitive for every program in the country so I would cast a broad net based on programs that have strong research in your particular field.
 
@Neuronix Do you think it would be helpful if I took a gap year for the reason of getting a publication (mid author)?
