Any real difference between this procedure and LASE?
 
This procedure is not endoscopically guided. It is more akin to the Choy procedure, however he uses a NdYAG laser.
 
CPT 62287. Reimbursement depends on the location so I would check on this. I plan on using in an office setting.

So it's basically $540 net 30 days + $78 fluoro fee (76005), or 76003-xx

I'll look at the literature and then see what the startup costs are before deciding on making the trek up North (only a few hrs drive).

The program went off without a hitch and the patient did well with the surgery. Cost of each laser fiber is $540 making it do-able in a hospital or ASC but not an office setting. Interestingly, the laser reaches a temperature of 800 degrees C and turns metal white hot, but it is a very focal laser with the energy dropping off very rapidly with 10% of energy present at 3mm and 0% at 5mm from the tip of the laser. Unlike the selective endoscopic discectomy or other arthroscopic discectomies, the procedure is performed in the absence of a saline irrigation.
Should be interesting for pain medicine if the pricing is right and if we can develop RCTs...
Thanks to Dan at NuSpine for organizing the meeting and thanks to Klaus for obtaining the FDA clearance, no small hurdle. It was great to finally meet lobelsteve and it was almost like having a reunion between long lost friends. He is a great guy and a very astute clinician.

Algosdoc is a true scientist. while he was building a laser as a hobby growing up, I was busy playing ball and chasing skirts. As a remarkable clinician, he is almost a cross between a surgeon and the rest of us. Algos- tools are for cars, not the spine.... we'll talk about how I can get some of that 😉 advanced training. Hey PAZ- no we cannot instrument the spine. Well maybe just a little bit. The laser demo was really eye opening. In untrained hands a single misfire will vaporize almost an entire root. The potential tissue damage would be immense. On the bright side, the patient didn't feel a thing during the procedure. The results from preliminary data put this laser ahead of other perc disc procedures. The science behind the laser is more sound than anything else I've seen or read about. Now about that Pain Medicine Residency.......

Yes, in some measure this procedure is quite simiar but PL3d offers important improvements to the previous. HFirt the wavelength used has 5 times better absorptiob by water and this neans less enery is needed which translates to fewer risks. The secon relevant improvement is that this technique has a built in smoke evacuation system which eliminates smoke generated during the process this translates also in fewer side efects and complications.
 
This laser creates a very pinpoint focus of energy that as we saw when the 15 watt laser was fired inside the needle, the metal became white hot. Now that is an impressive demonstration! Apparently 3mm away, only 10% of the max energy is realized, and 5mm away there is total energy dissipation. We need a RCT badly since there are none for laser discectomy even though there have been over 100,000 performed worldwide.
Steve is a real joy, and I look forward to seeing him as a national teacher soon...
 
Would you know the proper coding for reimbursement ----- both for the doc and facility? I heard that it is 62287 for the doc and S2348 for the facility, however I've also read that the procedure is considered 'experimental' and would not be reimbursed. Please offer your opinion.
