There's challenges with becoming a sub-sub-specialist. There's no official "Adolescent Addictions" or "Forensic Geriatric" spots when looking for jobs. Also, don't expect to learn how to integrate both fellowships during training, since sub-specialities are rare, it'll largely be up to experience and self-learning to be able to integrate the two.
There's several Attendings who have multiple Board Certifications due to the grand-fathering, however most focus on one of the specialties. Now fellowships are a big financial and time commitment.

Not sure what thread this was intended for. However, there are a couple attendings at my institution who have been able to carve a nice niche for themselves. One is double boarded in Child/Adolescent and Forensic Psych and regularly works doing child forensic stuff. Another is double boarded in child and addictions, and used to do work with adolescent substance dependence.

I have applied for child and, at some point in the future, will return to do geri/psychosomatic medicine fellowship. Not really for any career goals, but just because I enjoy C/L work
