Your description is too vague. For instance if you were accepted into neurosurgery but now want to do family medicine, this wouldn’t be an issue. If you were accepted into family medicine and now want to do neurosurgery, you have about a zero chance. You are somewhere on that spectrum but without specifics your guess is as good as anyone’s.
 
Should be doable then. Talk to your school administration about it and also go and meet the internal medicine and/or psych PDs to start networking. A MPH matters far, far less than your ability to network in this case.
 
Doing an MPH to be productive and extending your graduation for one year as a result is the opposite of productive. I wouldn't do it.
