As far describing experiences in secondaries, if you are not speaking about patients but only about an internship at a hospital or time shadowing a physician, are you allowed to say in the secondary the specific hospital you interned at/a physician you shadowed? Or must you keep in "a physician I'll call Dr. Jay" at "a hospital in ___ city" ?

Yes, you can mention the hospital name and physician you shadowed. In the Activities section of AMCAS you are required to give name and contact information, and location, as well.
 
You can use the real names of hospitals and health professionals. Of course, this'll eat up your word count if you do it too much, so only include names where it will make your writing clearer.
