NBME 16 question and UWorld Biostats Question

Hey guys,
I had a question regarding a biostats question from uworld and a nbme 16 biostats question.

Not sure if I'm allowed to post the exact question, so ill sum it up.
NBME 16 quesiton: determining the control group in a study to determine motor defects in children whose parents worked at a foundry. The Answer is apparently: First graders whose parents had never worked at the foundry.

Uworld question: hospital notes an increase in ALL in children. They think that the nearby factory increases the risk. What is the control group.
Uworld says that it should be: Patients from the outpatient clinic who do not have ALL. They say that based on the fact that to choose them based on exposure status, is bias.

So in the first nbme question, choosing the control group based on their exposure (parents haven't worked at the foundry), isn't that bias? how can it be a control group?

Im really confused. Biostats is one of the topics that always tricks me.
