All you have to put is your top prereq courses. For example if you did not do so well in a jr biology course then put a higher level biology course that you did well in. It would be unreasonable to put what you are already going to send in on your transcripts.
They usually take a test from one date but in reality the SATs are only looked at if there is a question about a course you had like English. They will go through your academic history to find out if it has always been a problem or if there has been improvement.

I guess don't sweat it since no one else can apply yet you won't be behind at all. Although I'm just as anxious to get it done with.
As for transcripts it looks like they don't want any high school ones...
"Transcripts:Please collect an official transcript from each college or university you attended. All transcripts must be originals issued directly from the institution and must be stamped with the schools stamp or seal. All transcripts must be issued to the applicant in a sealed envelope with the registrars official stamp or signature across the flap. Copies of transcripts or transcripts not sealed with an official stamp or signature across the flap will not be accepted. After collecting all transcripts, include them in the application package you will send to the Admissions Office at the New England College of Optometry."
