Need advice on future application and prospects; weird research/major combination and late bloomer

When are you thinking about applying? If you have bandwidth and it wouldn't be a significant financial burden you might reconsider taking the MCAT. Your score probably puts you at the bottom ~quartile of admitted students. Otherwise I think you have the potential for a strong application if you're able to clearly explain why MD/PhD makes sense for you. I'd encourage you to get started on the essays early to 1. Confirm this for yourself and 2. Allow time for several rounds of feedback

I strongly disagree. 516 is the average MCAT for all accepted MD/PhD matriculants in 2021. Thus 515 would not be at bottom quartile. Furthermore, given her/his URM status, as is, @Shorjand_U will be highly recruited by most MSTPs.
 
Your application sounds excellent and your stats (including MCAT) are great. Don’t worry. Lots of people do very basic science for their MD/PhD and it’s perfectly fine to come in without a clear idea of what you want to do. It is, however, import to justify why you want to pursue this training path.
