This is not considered a volunteer or community service activity, as it was for a family member. It could be listed under "Other" and the clinical relevance made clear by a good description of what you did. A rare adcomm member might consider it "clinical experience" but mostly it would be looked on as an altruistic activity. Fortunately, you have plenty of more standard clinical experience, so you're fine regardless.

It looks like you are pretty strong in nonmedical community service, leadership, and teaching. I think the lack of research will be less of an issue due to that.

The hobby and Arts involvment look good.

Any plans for physician shadowing?

I took all my non-science classes via extension studies
Does this mean they were distance learning classes via the internet? And if so, did you check to see if the med schools on your list accept such coursework? Have all your more recent classes, including some upper-level Bio and/or Biochem, been at a brick and mortar school?

How far are you willing to travel to attend interviews and go to med school? Loyola, SLU, and Creighton (no CC classes for prerequisites) would appear to be good matches for your ECs.

You might also check these schools for "fit" all of which have a decent OOS matriculation rate:

UCentral Florida, Miami, UConn, UMaryland, UKentucky, Rush, UIllinois, Jefferson, FIU (13% OOS, but expected to increase), FAU, Temple, LSU-NO, Vermont, Arizona X2, Louisville, and maybe some of the NY schools.
 
Also, what defines a "brick and mortar school"?
It means you were physically present in a classroom, on a campus, listening to a physically-present faculty member lead a class. This theoretically allows for a student to interact and ask questions better.

Another question to look into: Does your school transcript show that the course was distance-learning or does it not differentiate from classes taken on campus?
 
alltheabove, how do you expect to go about the caregiving thing? PS? Interview? I'm not sure how an adcom views such a thing. Based on my professors response (engineering profs) i'm not too thrilled with bringing it up at all.
