My friend:

step1 is 215

CS passed

CK will get score 3/10

Finished his resident and fellowship in home country for 11 years. USA research for 6 years. Good publications.

What's his chance? He heard it's almost 0.

Any suggestion?
 
Depends on the rest of his application, his step 2 CK, and whether he applies broadly and smartly, starting with his home program (depending on its tier).

If he wants it, he needs to do his due dilligence. And more important, he needs to prepare for the possibility that he doesn't match into rays (but matches into a prelim program).

Just to add re-reading your post... he needs to be careful. I take that your friend is an IMG (home country). Radiology tends to be notoriously biased towards US applicants. International applicants do get into rays. However, given your friend's stats, he is in for a bigger uphill battle than those who are US applicants with similar stats.

Keep this in mind.
 
215 is already difficult for US grads. Nothing is impossible but it's important to be realistic.
