Negative Aspects of Practicing In The Specialty?

I'm sure we all will come across this question during the interview season. I'm applying for internal medicine and the only thing I can think of is something along the lines of "when you fail, you fail big". However, I can see how this applies to EVERY SPECIALTY and absolutely not unique to IM whatsoever. Anyone have a better suggestion that won't pi** off the interviewer? My second honest answer would be "repetition", but I have a feeling that wouldn't go over too well either.
 
The classic complaint about internal medicine as well as family practice is dealing with disease that patients have brought upon themselves: smoking-related cancers and emphysema, obesity related diseases and the like. In addition a common frustration with those specialties is the difficulty in getting people to modify their behavior or even just to take their meds. Those are well-recognized problems and can lead to some interesting discussions during interviews.
