I'm an M4 and I had a hypertensive patient (200/100) last night that stated she "hadn't smoked crack in several days." When I saw her, she had multiple ocular nerve palsies, Right arm weakness and decreased sensation in her right side. I naturally thought she was having a stroke. My attending came over and with a lot of prompting was able to wake her up enough and said that crashing hard from Crack can cause these neurological manifestations. Sure enough, a couple hours later our patient was up and about and stealing the food off of other patient's trays.

My question is, have other people seen these type of temporary neurological manifestations in crack users before?

Usually I just pass gas, mumble, suck my thumb, and watch Jerry Springer until the feeling in my arms return.
But this isn't about me.
 
Where I did residency everyone was on meth. Yes, they seem to have this perios of autonomic lability while the high is tapering off. Then they do what I call the post crank big sleep. They will sleep, and be fairly difficult to arouse, for about 12= hours then they wake up and are ready to seek out more crank.
