Case 2: Elderly woman with SOB shown to have multifocal sclerotic blastic lesions in bilat femurs, fibulae and pelvis. Numerous consecutive biopsies showed only histocytes some of which had striated cytplasmic appearence on H+E. There was a minimal inflammatory infiltrate associated, no eosinophils. IHC shows these histocytes to be nonLangerhans type, CD1a and S100 negative but positive for CD68.

Diagnosis:

PS-This is an insanely hard case, in fact Im like one of less than dozen in the US who have made it. 42 reported cases worldwide. If you get this, skip residency go right to private practice. Surg path directors at 3 major hospitals couldnt get it.

Erdheim-Chester disease or if you prefer polyostotic sclerosing histiocytosis.
