I have a middle aged female patient who describes pooping out "little mushrooms" ... little round, nodular balls in her stool with a soft inner core like a mushroom. ... what the what??

After begging for clear descriptions, she took these pictures ... I remain stumped. We've exhausted our questions to her, and nothing really stands out in her history.

Anybody seen anything like this??

I think she's lying to you. Those are likely actual mushrooms, or some other foreign object. She never pooped them out at all.

Seems probable. Maybe Munchausen or some other mental illness? Not too many people are willing to dig through their poop to fish out and clean up some strange objects. So some or all of the story is likely fabricated.
