So I have a gap year job lined up and put it in my work/activities and marked the future hours. But there is a chance I may get a new better gap year job after submitting primaries. Do I just keep my confirmed job on the app for now and if it switches can I notify them later? Both are scribe jobs so actual clinical hours shouldn't be that different but the contacts will be different. What are your thoughts on this?

Adcoms understand that plans can and often do change after submission, which is why future work/activities listed in AMCAS carry very little (if any) weight. If someone felt the need to verify this activity (they won't), you can give them the updated contact information at that time.

Only go with what is actually happening, not what might happen
But what actually happens might be subject to change just 2 weeks after my primary is submitted. Anyways I'll just put the one I have confirmed and I guess they'll be understanding if I switch to a different job?

Things change.. Just be truthful to what you expect to be working as and how much you will be working.. They understand and they know that things change.

This won't be a big deal, as you're changing venues, not the essential job description. Many Secondary prompts will give you the opportunity to update your planned activities. Update letters, where allowed, are another option for letting schools know your job location changed.
