It's like anything else. It's a club based on networking and who you know and who is willing to play the game. Same for the podcast and social media circuit.
Maybe, maybe not. This year I didn't even apply and was selected to give two talks in the actual conference. I'll also be doing the evening event for the third time. It's not really much of a game though, there's no money in speaking at the actual conference. You get free admission and expenses but no speaking fee. I kind of find it interesting that I could run a conference, feed the attendees, pay my speakers very well, and still realize a pretty good profit. I guess all the money you pay for the conference goes to fund everything else ACEP does, dunno. Probably would be interesting to dive into the finances of most professional societies.
