NHSC scholarship, "underserved area?"

1) are you able to choose where you want to be employed or do they locate you?
2) anyone have any examples of "underserved areas" -- I'm sure many of these are rural sites, but I'm hoping there would be a few urban locations as well.
3) This starts after a primary care residency?
