So for anyone who might know, how will the elimination of the out of the match positions work? I mean say after the scramble that positions are still open, no one will be able to take those? Or what will happen when a position becomes available either via a resident dropping out of a program, getting fired, etc. Will that program have to wait until the match or will those positions still be available out of the match?

This only applied during the match ...sept-march... the rest of the year is fair game.

The new rules mean that if a program wants to participate in the NRMP match, ALL of their spots must be in the match. Previously, programs could hold some positions "outside the match." The scramble would be unaffected, since those spots were included in the match. Vacancies wouldn't be affected by the new rule.

Right, I realize the scramble would not be affected. But let's say for example that it's June and someone drops out of a program, that spot is suddenly open and someone else says hey, i like that specialty, I'd like to take their spot. Can that be done? Or does that mean that the program will have to wait until the next match to fill that spot?

I knew that some programs made "prematches" if they were concerned that the program would not fill, but did not think it was that widespread overall.

The program would still be able to fill that spot. The spot was originally in the match, it wasn't like the program was holding a spot outside the match.

Towards the end of this presentation (2nd to last slide), the NRMP addresses exceptions to the "all-in" rule:

Possible Exceptions
“accelerated” programs, e.g. Family Medicine
rural scholars programs
combined clinical-research programs where the first year is not clinical
IMGs with funding from their home countries
programs in rural and geographically underserved areas
resident transfers to a different program in same specialty
mid-career specialty changes
off-cycle residents, where training does not begin in July
dually-accredited programs
partially matched applicants at end of SOAP

I saw some of those exceptions, I guess I did not see the rest about off cycle residents, partially matched residents, etc. Thanks man! 🙂
