No PI letter due to reporting for ethics/safety: what to do about reference?

You can ask the supervisor you reported the incident to to serve as your reference. If by any chance (which is unlikely) the medical school decided to contact the supervisor to verify your research position, they'll be able to confirm that you did indeed participate in this activity.
 
I just wouldn't ask for a letter from anyone related to this. I worked in a lab for one semester and it ended poorly with the PI ( I also took it for credit and had a class with him). I didn't get a letter from him and during interviews when asked about my research I just said I wasn't interested in basic science, but that it was a valuable learning experience and that clinical research was better for me. no one batted an eye
