My friend recently received the 3yr HPSP and signed on for a 4th year to receive the $20k signing bonus. She now regrets it so I was wondering if there was a way to give the money back? If not, what would happen if she left the army early and didn't serve her 4th year? (Her recruiters don't know so I've turned to SDN)

I believe their is a specific calculation of financial penalties in the paperwork....but they can literally send an MP to pick you up of they want

It's called unauthorized absence, and usually results in confinement. Once you sign a binding contract like that you don't get the option to leave whenever you want.
 
Only the government gets to decide when to allow someone to back out early and pay back on obligation financially versus through service. The dentist does not have that option to pick and choose.
 
This is a cross post and the answer is the same in both places: Have your 'friend' read her contract. If she does not have a copy she can contact the HPSP office at 877-MED-ARMY and they will provide a copy to her.
 
Why is your "friend" regretting it? Just curious.. Did she win the lottery?
Taking the bonus and incurring an additional year doesn't make financial sense whether one is staying in or getting out because staying in you can sign the multiyear retention bonus a year earlier which is more than 20k or getting out where you would make more than the standard military pay plus

Unless you need 20K heading into school. That money goes a lot further when you're living on stipend than it does when you're out.
