Not sure what's going on with my LOR

I've shadowed a local dentist for about a year and a half (Mid-2015 to early 2017). He and I have had "the talk", in which he would write me a LOR if I wrote him a paper about the effects of tooth infection.

Unfortunately I couldn't shadow with him for about a year because of school and other personal issues. I came back, shadowed with him, and let his staff know that I was going to send him the paper the next day (I didn't get a chance to talk to him about my LOR while I was shadowing; an incident happened in their office and I didn't feel like it was an appropriate time). I sent the paper 3 weeks ago, and I haven't gotten a reply back. I ended up going there, and while I wasn't able to see him, one of his staff went to him and he seemed confused as to what paper I was talking about I was instructed to send the email to how work email, and I still haven't gotten a reply. It seems like all my methods of contacting him aren't working, and I don't want to come off like I'm bothering or pestering him. I have less than a month left before I send in my application, should I find another dentist to shadow or should I keep trying to contact him?

Call the office and find out which day the dentist has an "easy" schedule around lunch time. Ask if you could bring lunch for the dentist that day. If you can swing lunch for the entire staff .... that would be a nice gesture.
