I occassionally use a ntg bolus (25mcg or so) in cardiac, for cannulation etc. We have a ntg shortage and are being supplied with nipride instead. Does anybody know the exact potency conversion? I've never tried to bolus nipride. Thanks.

Small propofol (40-80 mg) bolus works pretty darn good.

Our preparation comes as 200 mcg/cc in the drip. I will dilute it out in a 12 cc syringe to 50 mcg/cc (3 cc nipride with 9 cc saline). 50 mcg nipride will dump their pressure significantly (sometimes 80-100 points, but usually 40-50 points) for cannulation/proximals/XC/etc. It goes away quickly, like a minute or two. very useful.

This is what I used to do when I did hearts. I would test with 25 some time after induction to see how dramatic the drop would be.

There isn't really an exact potency conversion per se, but I find 25 mcg of nitroprusside to be a pretty average dose. I prefer it as I feel it is more predictable than NTG. Obviously the mechanism is quite different and I prefer it's mechanism.

I don't do cardiac anesthesia, but from experience I've used nipride in 25mcq boluses for pheochromocytoma cases. The effect is quick when there are dangerous spikes in the BP. The effect goes away pretty quickly, so I've never had an issue overdosing patients.
