Could you guys give me an idea of roughly where I should be targeting in terms of IM programs?
Congrats on ur strong scores. Ur research is lacking but it'll be fine for most places. Are your letters from faculty from university hospitals? or are these community preceptors? If the latter, you need to do an audition or elective to get some university letters. they can carry some weight as long as you bust your butt on them but don't act like a gunner.
I assume places like the University of Michigan are out of reach no matter what I do, but what about somewhere like USC or the University of Chicago? Is that a realistic target?
None of those programs have accepted a DO into their program for any PGY year for any of their tracks recently. Sure you can throw an app there and you may get an interview but don't expect to be ranked high enough to match there since they get a HUGE swath of USMDs from T20 with applications just as good if not better (along with the MD behind their name). If you think you have a good shot at one of those places by connections I would def try and do an audition there. That could help.
Apply 30-40 with 20-25 university, and 10 safety community.
Maybe 2-5 could be reach like the ones you listed. But you'll never know if you don't apply. Just don't expect anything.

My understanding is that Dartmouth isn't considered an elite place for residency training in most fields. Now that isn't to say it isn't a great place to train.

To date the best DO IM match that I am aware of is UT Southwestern and from what I've heard through the grapevine that person had 260+/270+ and a solid research CV.
They had a 25X/25X, but yes, an excellent research CV and matched into the research track/physician scientist program.
To OP, any program that has taken DOs in the past (which is most non top 20ish programs--check on Freida, Residency Explorer, or worst case the program websites themselves) is probably reasonable to shoot for with your credentials. Based on my friends who had similarly impressive applications this year, academic programs in very competitive geographic areas like California and NYC are going to be hard to break into even with your scores. For example, none of the big 4 NYC programs are going to look at you, except NYU's Brooklyn/primary care track. Same with UCSD, UCLA, and UCSF. In the Northeast, places in Philly (not Penn), the DC programs, Dartmouth, and Brown all interviewed classmates of mine. The Midwest and South will be even more accepting, but depending on where you're from you may see some geographic distribution of your interviews. I very clearly have a lot of Northeast ties in my application and I barely got any interviews outside the NY/NJ/Philly metro areas. My classmates from the midwest got interviews across the country. The best match in my class was a top 25-30 program, at a program which (rarely) took DOs in the past. He had slightly worse scores than you but a similar app.
My advice would be to apply broadly unless you're geographically restricted, but to make sure you have some safeties just in case. It's probably reasonable to send some sacrificial applications to places like U Chicago that have never taken a DO before and see what happens, but make sure to apply to a reasonable amount of mid-tier places as well (U Minnesota, UIC, Georgetown, Jefferson, Wake Forest, etc). Personally, I have yet to hear of audition rotations helping anyone this year in IM among my friends--some people got curtesy invites where they had rotated but none of them matched there despite putting them #1.
As a final note, I'm really not convinced there's as big a difference as SDN says there is between top 10 programs and top 50-70 programs as far as your fellowship/career, unless you really want to do academic grant funded research or impress your grandma. Not that you shouldn't shoot for the best program you can get, but just to keep in mind as to not get too caught up in the prestige game that is applying to residency.

Would you mind telling me which program you are referring to here? If you would prefer not to publicly state it, you can also message me directly. I really just want to compile a list of places I should begin considering and would like to know more.
