FYI I wrote to Governor Cuomo this past weekend after a friend of mine had a wonderful suggestion to allow IMGs who are unmatched, studying for exams, or not practicing medicine yet in the USA to be able to provide additional staffing or help for hospitals in need. This is for IMGs that have had previous training or residency in their home country. Often these are capable Doctors who can provide great support for understaffed hospitals full of Covid19 patients.

Today this was enacted in NY with the same terms I had proposed!!

So reading that, there's a smorgasbord of stuff included.

Allows NPs, PAs, "specialist assistants", CRNAs to practice without supervision. Makes all of them (+ physicians) immune to liability for COVID related stuff. Allows students "in programs to become licensed" to volunteer for educational credit without a formal agreement. Allows lax recordkeeping. Removes all resident work hour restrictions (the NY legal ones from the Bell commission days presumably - the ACGME ones are likely still in force). Etc.

The IMG thing isn't clear though. This is the relevant paragraph.

Subparagraph (ii) of paragraph (2) of subdivision (g) of 10 N.Y.C.R.R. section 405.4, to the extent necessary to allow graduates of foreign medical schools having at least one year of graduate medical education to provide patient care in hospitals, is modified so as to allow such graduates without licenses to provide patient care in hospitals if they have completed at least one year of graduate medical education;

Without another source that says they're allowing the practice *with only foreign gme*, I think all this does is lower the requirement for independent practice from 3 years (which is the NY requirement to get a license as an IMG) to 1 year (which matches the US grad requirement). But on my reading of the plain text it would still require a year of US GME.

Basically, if you did one year of residency in the states and your a foreign medical school graduate, you can get your license instead of doing at least 3 years of residency. Imgs who are not US citizens or residents will still have problems with hospital credentialing due to visa restrictions. And at this point with many government offices closed that would endorse green cards or issues visas, it will be a struggle.

Yeah, not sure this is good. Every residency-terminated doc has a chance to work unsupervised when their lack of skill/knowledge may have been what got them terminated in the first place (thinking of Stephanie Waggle). What are specialist assistants anyway? Like PA/NPs who are in specialties?

I would assume field-specific assistants. Like there's "anesthesia assistants" in some states which are PA equivalents but specifically trained for anesthesia only - kinda like a CRNA is an NP equivalent. I don't know how many fields have similar.

From what I vaguely recall of her case, Waggle and similar may be able to get independent licensure anyway - isn't she a US MD? She did >1 year of residency, and I think she got credit for her intern year - so she can be licensed in 30-something states.

Edit: Yeah, she's licensed and hung up a shingle to do therapy on her own. No idea how that's going for her.

So what does this mean exactly? Is this all a temporary measure? Would these foreign grads be granted a license to practice? Will they keep the license afterwards? Or is this basically just temporarily removing the license requirement to practice?

Yeah, which in and of itself is wrong. I shudder to think that if I get sick, someone like her can treat me.
