"[Is there] any information you wish to bring to the attention of the Admissions Committee regarding a physical or emotionall problem that you feel may have affected your scholastic performance?"

I am not sure if they are asking about an actual diagnosed emotional problem like severe depression or something or if they'd like to know if you were in distress for some reason. For example, if your performance was affected by some family issues, would you put it down?

I would think that family issues would be emotional in nature. If you feel that your grades were adversely affected, go ahead and use that space on the app.
