Does anyone know what's on it? They said that they would give it out when I arrive at the interview? Is it more just information based? Or is it actual tough questions like ethics or something?

No. It asks for your address and other schools you applied/interviewed. It takes about 5 minutes to fill out and you do so while you are waiting for everyone to arrive/presentations to begin.
