The offshore MD route will be longer than PA route due to a residency needed. Offshore MD programs are not competitive in the US, so you would need to realize that your options may limit you to primary care in a poor program if you can land anything at all.
With residency positions not expanding and US MD graduates increasing, offshore MD applicants may be in for tough times.
I'd consider the PA route hard.

The most practical idea will be the PA road.
But in my opinion will you be satisfied with the PA road ?
Go the MD road even if it take you years. At least you will have accomplished what you started years ago.
