Does anyone know what exactly they're looking for with these reports (i.e. content, format, length, etc.)

I thought it was pretty self explainatory. Just download the form they have online and have the dentist fill it out and submit it to OSU... That's what I did.

well, from what i read from the description, it sounded as if we had to write an actual report along with the downloaded form.

SO I guess the only thing we have to do is turn in that downloaded form?

If so, hey, less work for me.

This is the gist of what they are looking for and what the form entails - you fill it out and have it signed and mailed by a dentist that you shadowed. There's nothing really to it.
