I am going into, or plan on going into derm/internal medicine combined program. I was wondering if once you are accepted, if you can do that first year...(i believe the first year for all four programs that exist revolve around internal medicine)...in another university or community hospital. I've always wanted to go to california and would love to do that first year there. Or are residencies strict and only allow one to do the 1st year in their program once they are accepted?

It depends on the specialty and the program. Most derm programs require a year of medicine prelim, but some may require that you do the prelim year at the same institution. Others may allow a prelim year anywhere, meaning you will need to apply to a separate prelim year and match there (which means more interviews, etc).
