I'm going to look into that book the other reply mentions, but I also find it helpful to simply take already-dictated op notes, shrink them down on the copy machine (WAY DOWN) and glue them into a little leather notebook I carry around. At a bookstore, I found a "moleskin" notebook that is just about the size of an index card. It's where I write my little 'pearls' that I need to remember about attending preferences, etc. Interesting history, it was the kind of notebook Hemmingway carried around with him on his travels and wrote stuff down in. So, they are back en vogue. It has an attached elastic binding that holds it shut and fits great in my pocket. In the very back, I've taken a gluestick and glued dictated opnotes. I underline in red the stuff that needs to be changed during my dictation. I've also minimized and glued in charts from uptodate....pressor charts, hypertension drug chart, insulin chart.
