Does anyone know any good opiod conversion sites/books? This has always been a struggle for me and I don't feel like I learned it very well while in fellowship.

i have found this hopkins web-program helpful. The used to have a PDA version, but I wasn't able to download it and now that PDA are by gone I don't think they ever updated that program.

By the way where did you go for fellowship?

i dont convert opiods much b/c i dont write em much, and i have some poster thing on the wall, but i can tell you where i got that thing from monday.
just remember to 1) decrease the dose by a third whenever you change from something, 2) always start at 5 mg of methadone regardless of the conversion, if you choose to use it (and i dont) 3) use dilaudud 5:1 for morphine NOT 3:1, 4) IV to PO is 3:1, 5) IV to epidural 1/10 the dose, epidural to IT is 1/10, 6) PO to IT is 1/300th and you will stay out of trouble, hahah
