One of my attendings in residency had very old photo of him doing an orbital intubation in the 80s/90s. Said he used a miller 3 and it was a literal straight shot through the cords.
As someone who has asked about simple retrobulbar blocks via anesthesiology on here and in various ORs, and who has gotten basically a straight up no/never, WTabsoluteF is going on here

A little off-topic but this is one of the authors.
Interesting read of that lawsuit details, doc sounds absolutely insane.
I thought I was cool because I did a couple submental intubations…
Ron Miller during my interview for residency, recounted how he performed an orbital intubation in the Vietnam war. Pretty cool...
Looks like they’d had an exenteration, so everything was already out of the orbit at least.
To quote my cynical oculoplastics mentor - “No eye, no problem.”
I mean that's the point, he couldn't see it coming!

This is crazy. Simple nasedoscopy or using those special soft nasal tubes would have been a far better solution. @ least with the nasal route its common and theres txa and ent. You screw around in the orbit and its whole different ballgame surely?

I stick sharp things into eyeballs for a living. Thousands of times a year. Somewhere I have a nice video of me taking a fishhook out of an eye that scares pretty much anybody who’s seen it. This manages to terrify me.
Lol- if the other eye is working, it’s almost worse, bc he WILL see it coming, but will have no depth perception, so he can’t even judge when impact will happen! 😅
