Does anyone know if you can order a CD4 count on step 3? I'm playing a guessing game trying to find it but I don't think its there. Any suggestions?

I think you have to order AIDS T Cell Profile.
Then it will show up.

To see if any particular study is on the actual test, and to see what phrasing they utilize (they use several variants for the same test), take the practice CD and order it on the test patients. Even though it most likely won't apply to that case, you can start to see the list the USMLE Step 3 folks utilize for the real exam.
