If you're actually on an ortho rotation now, and it's the first time you've done one, I doubt the residents will expect you to know too much. Ask them what they want in the note and H&P. It can be sort of school specific. (My home program, AVSS usually suffices for part of it, and at my away, they wanted all the vitals written out).

here, a ortho student's soap note is something like this

pt resting comfortably. pain controlled.
afvss
R UE: 5/5 ain/pin/u SILT r/u/m BCR
dressing c/d/i no soi at pin sites
cont pain control
cont abx


alternatively, the PE may be shortened to "NVI" after POD 1
