Aren't you still a predent? If you're so inclined, go to the PASS website and check out what's on there since the majority of programs participate in PASS. www.adea.org/pass

The applications are all usually the same - name, address, schools you attended, who is writing your letters of rec, GPA, answer a bunch of nonsense about stuff that probably already on your resume, and essay.

the answer to your question is complicated
many schools participate in PASS - on www.adea.org (where AADSAS was)
many others have their own applications as downloadable pdfs

the only way is to check each website individually (yes that sucks and takes months)
