Can dually accredited (allo and osteo) residency programs offer prematches to osteopathic applicants if they are "all in?" It would seem unfair if a program is "all in" but can still offer prematches to osteo students and not allo students. Can anyone confirm?

No prematches. They can offer the spot in the AOA match and let it be filled that way though.
 
This specific issue is addressed on the NRMP site as a FAQ:

4. My program is dually-accredited. Do I have to place all my positions in The Match?Programs accredited by both the ACGME and the AOA will register some positions in the AOA Match and some in the Main Residency Match. If the program reverts to the Main Residency Match any positions not filled in the AOA Match, the program will be subject to the Match Week requirements in Section 7.0 of the Match Participation Agreement for Applicants and Programs.

So, yes, DO students can prematch into the spots in the AOA match if they do so before the AOA quota deadline. All of this should go away when the AOA match is merged with the NRMP match, an assumed outcome from the merger of the ACGME and AOA.
