Do we need to report all previous addresses? Is it important that I find the correct dates of residence?

I have moved at least 6 times, so this is quite difficult.

It doesn't seem like its too terribly important - I put down the 4 addresses of where I've lived, and once the check was complete, I noticed that Certiphi inlcuded 3 versions of the same address (ie, address, address + unit 2, address + 9-digit zip code instead of 5) and even included my dad's business address... It wouldn't hurt to include them.

I actually ran out of space on the addresses before the time backwards was reached. You should try to include as much information as possible. I looked at old documents, etc. to get the previous addresses.
