USMLE Out of time and not ready for Step 1. Have I screwed up?

Hello all. I'm a DO student and have recently taken COMLEX Level 1 as of last week. I am scheduled to take USMLE Step 1 on July 14 as well, but I have a problem - I really don't think I'm ready yet. The problem is, I can't push it back any more, as I'm supposed to start clinical rotations on the 21st.

I really don't feel ready for it because I haven't been getting consistent 60% or better scores in UWorld. I also haven't done super well on the NBME practice tests; I've taken two so far and the results were a 92% and a 72% chance of passing, with both score percentages in the barely-passing range.
On top of this, I feel burnt-out and in a mental fog. I've been resting quite a bit since COMLEX and took some time off for family stuff, but it doesn't seem to have done any good. I don't feel more able to get back into things, and I'm not getting the scores I want to see. I don't feel ready for Step 1.

I'm running out of time. I feel like I'm set up to fail this exam at this point. I don't know what to do. Have I screwed up too badly?
