Do you have any policy regarding patient forms? E.g. for disability or FMLA etc.?

Is it legal to charge for these things?

Yes, it’s legal. We may charge a form fee of $10 to $25 for forms completed outside of an office visit, depending on the form. Complex forms typically require an office visit.

I’m employed, and am not set up to charge a fee. So I require an office visit for any and all forms. I then code on the problems the forms are directly related to.
