Freaking out after applying accomplishes nothing, so maybe consider going back to being chill. There is nothing to do now except explain it and show growth and contrition when prompted to on secondaries.

Yes, competition is intense and 2 IAs in the same year for the same thing is not good. The first was intense youthful indiscretion (not merely drunk, but too drunk). The second in the same academic year is harder to explain away and very well might cause some highly competitive schools to pass on you. No way to tell until it happens, so, again, what's the value of stressing 2+ years after the fact?

Schools see IAs whenever they choose to see them. It's not like AMCAS dribbles out various components of an application over time. It's all electronic, and schools can slice, dice and screen applications however they see fit. Some schools will screen out anyone with an IA before even sending a secondary. Most don't, and happy collect secondary fees before doing any screening at all.

Once you are complete, I don't think there will be any delay due to reporting 2 IAs. Some schools will look past them because they love the rest of your application, and others won't. It is what it is.

You are correct, getting into med school is extremely competitive, and mistakes can be fatal. Engaging in under aged drinking to the point of needing to be assessed by EMS is not really the "smallest mistake" you'd like it to be, and you'd probably be well served not to frame it as such in your explanation.

Having yet another IA for an incident related to under aged drinking during the same year might be even worse, because it shows that you were not "scared straight" by the first incident. It also calls into question how many incidents went undiscovered. Substance abuse is definitely a hot button issue in medicine.

I'm sorry I don't have better news. Yes, this will probably hurt you at some schools, but it is extremely premature to write off your application in June. Please don't waste energy obsessing on something you cannot undo. Just focus on an introspective explanation, and allow things to play out, since it's all you can do at this point. Good luck!!
